Responsibilities:

• Triage all incoming phone calls for Hospice Team.
• Assist in referral management by gathering required information, updating referral tracker and working closely with the Director or Nursing.
• Assist in ordering Durable Medical Equipment.
• Work closely with all hospice team members to ensure compliance with required patient documentation.
• Responsible for keeping new admission paperwork and supplies prepared for Hospice Nurses.
• Compiles and organizes information for Interdisciplinary Team meetings,
• Complete referral/intake forms and route them appropriately for admission approval.
• Ability to work in a fast-paced environment and be flexible.
• Perform other duties as directed.

Requirements

• Experience in a Hospice setting preferred.

• Quality Assurance experience preferred.
• Must have excellent computer skill - Excel, word
• Possess and orientation to serve
• Excited to join a growing team
